I affirm that I have legal authority over the Child listed above and that child has my permission to participate in any/all of Immanuel Baptist Church Vacation Bible School activities and that He/She is in good health and has no condition which prevents said participation. It is agreed that neither Immanuel Baptist Church, nor its volunteers or staff assume any legal liability for injuries or other losses from participation in Vacation Bible School. I understand that in some of the activities injury may occur from participation. I hereby give permission for my child’s photograph/image or likeness to be used for promotional purposes

As the parent/legal guardian of the child listed above, I hereby give my consent for emergency medical care prescribed by a duly licensed doctor of medicine or dentistry and that said care may be sought for my child by and at the discretion of Immanuel Baptist Church and is volunteers. This care may be administered under whatever conditions are necessary to preserve his/her life, limb or well being of my child.
